Pulling from Shawn’s musical background, the term, “Duet”, only seemed befitting of his vision for a direct primary care practice. Defined, Duet means a coordinated, synchronized effort between two performers that complement each other to create a single, harmonious, work.

At Duet Health, we don’t believe in call centers or waiting rooms. How many times have you been on hold for ten, twenty, or thirty minutes when you’ve called your doctor’s office to ask a simple question or get an acute appointment? And when you finally get that appointment, how often has it been scheduled with a provider that you’ve never met before? At Duet, you’ll never be bounced among multiple people before you can actually speak with your provider – you will only have one provider, and you’ll have their cell phone number. And we don’t need to rush you through your visit in order to see more people. All patients get the time they need (our average appointment times are 30-90min, not the 15-20min that you’re probably used to).

• Expanded access appointments (30-90min) when you need them (early mornings, days, evenings, and weekends)

• Unlimited visits

• In-home visits when absolutely necessary

• Direct line of communication to your clinician (with timely responses)

• Virtual (video) visits using a secure app

• POC (point of care) testing: rapid strep, flu, urinalysis, glucose, etc.

• Access to heavily discounted lab services

• Pre-operative risk assessments (medical clearance), including EKGs if required

• Physicals, chronic disease management, hospital follow-ups (ER or admissions)

• Cannabis card certifications

• College/Sports physicals (also available for nonmembers for a $100 fee)

• Acute/Urgent attention such as musculoskeletal injuries, respiratory infections, UTIs, rashes, skin infections (also available for nonmembers for a $175 fee)

• Basic mental health monitoring and medication adjustments

• Electronic prescriptions

• Coumadin management

• PReP surveillance and testing 

• Ear wax removal (cleansing)

Although we expertly perform and provide the following services, additional fees may apply for nonrenewable supplies such as heart monitors, suture and procedural kits, injectable antibiotics, scalpels, punch biopsies, and nebulizer supplies. Your charge will be our cost (plus 10%) and applied to your credit card on file.

• Wound care, skin biopsies, foreign body removal (including eyes), cryotherapy for skin tags, precancerous lesions, and warts

• Laceration repair, I&D of skin cysts and abscesses

• Heart monitors

• Phlebotomy on-site, or mobile (have labs drawn at your visit rather than going to a lab)

• Injectable antibiotics, B12 injections

• Nebulizer (breathing) treatments

• Vaccines. We are “vaccine neutral”. We do not provide any vaccines. If you want our advice on vaccines, we are happy to have an open and honest discussion about the risks and benefits.   

• Gender affirming care. We do not provide hormones or perform pre-op risk assessments (medical clearance) for any gender-affirming surgical interventions or procedures. We are happy to refer you to a mental health specialist if you are struggling with gender dysphoria.

• Female care – such as pap smears, pelvic, and breast exams (we hope and expect this to be temporary). This is simply due to lack of female chaperones required for these types of exams.

• DOT (Department of Transportation) exams

• Chronic pain management requiring or involving the use of controlled substances.

In a traditional primary care practice, clinicians are compensated based on how they code and the quantity of patients they see, rather than based on the quality of care they provide. In a DPC practice, quality of care is the number one priority. By cutting down on the panel size of patients from approximately 2500 patients to <400, providers working in a DPC practice are able to spend additional time with patients; patients have easier access to appointment times that meet their scheduling needs; patients are guaranteed to see their clinician, not another provider; patients are able to reach out to their doctor, PA, or NP via secure email and cell phone—directly.

Duet Health does not accept or bill your insurance, after all, we work for you, not the insurance company. We charge a flat monthly fee which covers most primary care services. We save money by not being contractually obligated to charge inflated insurance rates. Patients should still retain an insurance policy in the event of catastrophic illness or hospitalization, or to see specialists if the need arises. Many people find a high-deductible insurance plan with a lower monthly premium, pairs well with Direct Primary Care. 

If you need to be admitted to the hospital, we may visit you to help coordinate care, but our membership will not cover the cost of your stay or the care from the inpatient team.

Being a member keeps down out-of-pocket costs (such as charges applied before you meet your deductible). This allows you to purchase less expensive insurance, while enjoying superior, direct, and personalized care from your primary care provider. 

Although we can offer deeply discounted rates on most labs studies, you may choose to use your insurance for these services if you prefer.

Direct primary care is an alternative and better model for primary care. We don’t believe it makes sense to use a fee-for-service model for primary care. Clinicians who practice primary care in a traditional insurance-based model are forced to see an unsafe number of patients at shorter intervals. They must then document the details in a medical record with the hopes that they’ve met the insurance company’s payment criteria for reimbursement. If your insurance covers primary care, you are paying for an expensive benefit that is often difficult, if not impossible, to enjoy. Direct primary care is more comprehensive, highly effective, and less expensive than the current insurance-based primary care model.

Even though we are a member-based practice and do not work with insurance companies, that does not impact our great relationship with trusted members of the surrounding medical community. We partner with labs, hospitals, and specialists, so we are able to coordinate and advocate for you and your health needs, and will work to break down any barriers that can make your health care coordination difficult. We are not rewarded by the government or commercial insurance for how much or how little we do for you, nor do we refer exclusively to a particular hospital system, so you can be confident that we have your best interest at heart. We’ll refer you to who we feel can best meet your medical needs.

Yes! Federal legislation was passed in 2025 which took effect January 1, 2026, allowing you to use up to $150 per month for an individual membership (a maximum of $300 per month for a family). At Duet Health, we accept your HSA card as a payment method.

DPC practice can take care of 90% of the healthcare services an individual needs for a fraction of the cost. Insurance can have high deductibles with large out-of-pocket expenses for co-pays, facility fees, co-insurance fees, and deductibles that must be met.  Other than your annual physical and basic preventative screenings, everything else you pay is 100% out-of-pocket until your deductible is met.

Early studies show that a DPC model results in 20% lower overall healthcare costs when compared to a traditional FFS (fee for service) model with overall increased patient satisfaction. This is believed to be related to the direct access that patients have with their provider, the time the provider spends with the patient, the opportunities for earlier intervention to address acute and chronic issues, the time to help educate, and the DPC model being able to reduce ER and urgent care visits. Patients that have a DPC provider have also been proven to have reduced hospitalizations with shorter stays.

New Hampshire House Bill (HB1222) was passed in 2024 which now allows PAs who have >8,000 hours of clinical experience to work independently, without a collaborating physician. The PA must submit a formal waiver request to the Board of Medicine. Shawn has >55,000 hours and received his waiver in the summer of 2026.

Our service contracts are for one year and commence on the first day you submit payment for your first month’s membership fee. If for any reason you wish to cancel your contract, you may do so with a 30-day notice. All contracts are automatically renewed at the conclusion of the twelfth month. Duet Health reserves the right to terminate your contract for any reason, but must also provide you with the same 30-day written notice. After contract termination, a 90-day supply of your current active noncontrolled substances medications will be sent to your pharmacy. Controlled substances, such as medications for anxiety, pain, or ADD/ADHD with be sent in for a quantity sufficient for a 30-day supply (will no refills).
